Application Process

The application process starts with talking to a nurse at Integrated about the kind of help you need in the home. These nurses can’t approve you for the Medicaid Waiver program; they can only discuss the eligibility process and steps you need to take.

After you talk to the nurses, you need to complete a referral form. You can click the link below to print the form, or the nurse at Integrated Resources can send you one in the mail.

Once you have the form, fill in the top half of it. Then take the form to your doctor to have the bottom half completed. Please pay attention when you take the form to the doctor’s office. Many people see nurse practitioners or physician assistants—they can’t sign the form, it has to be signed by a doctor.

When the form is filled out, it will be sent to a company in Charleston called West Virginia Medical Institute. A nurse from that company will call you to schedule a home visit. During that visit, the nurse will see what kind of help you need in the home and determine if you are medically eligible for the Medicaid Waiver program. If you are, you will receive a packet in a few weeks that has forms that need to be filled out and sent back in. If you don’t understand the forms, or are unsure what to do, you can call the nurses at Integrated Resources to discuss them.

After those forms are sent back, you will have to complete a financial review at the local DHHR. They will look at your assets and monthly income. Right now, you have to have less than $2000 in assets and have an income less than $2022 per month.

If you are financially eligible for the program, you will start receiving services soon after the financial review is completed.
